To complete registration, the following health documentation must be uploaded to Aspen during the registration process:

Physical Examination: An up-to-date physical exam (completed within the past 12 months) for all new students.

Immunization Records: Proof of required immunizations for school entry. Please click here to review the current Massachusetts School Immunization Requirements.

Lead Screening: Proof of a lead screening test for students entering Kindergarten.
